Medical Social Worker
Assesses the social and emotional impact of the program on the patient and his/her family.
Assess the patient and family with personal environmental difficulty in order to estimate their capacity and potential to cope with problems of daily living.
Participates in the development of and makes suggestions to the physician for additions to the Plan of Care. Helps the patient and his/her family to understand the medical team’s activities.
Works with family to assist in utilization of appropriate Comprehensive resources or to live within the disability.
Participates in discharge planning and in-service programs.
Acts as a consultant with other members of the health care delivery team.
Assists client and family in understanding, accepting, and following, medical recommendations and provide services planned to restore the patient to optimum social and health adjustments within patient’s capacity.
Observes, records, and reports social and emotional changes.
Assists in preparation of periodic reports sent to the physician and third-party payers regarding care given.
Attends in-service as required by Agency policy.
Provides liaison contact and follow up with other Comprehensive agencies.
Coordinates services with agencies or facilities already involved with client and family.
Coordinates referrals to other Comprehensive agencies as indicated.
Attends case conferences and staff development activities.
Assists in development and revision of the physician’s Plan of Care.
Submits assessments and progress notes in a timely manner, as required by Agency policy.
Supervises Social Worker Assistant according to Agency policy.
CPR is recommended but not required
CRITICAL DEMANDS
Visual/hearing ability sufficient to comprehend written/verbal communication.
Ability to deal effectively with stress.
QUALIFICATIONS
Master’s degree from a school of Social Work that has been approved by the Council of Social Work Education.
ACSW Certification or be in the process of acquiring certification is preferred, but not required.
Licensure by the State, if applicable.
One (1) to two (2) years post graduate Social Work experience in a health care setting.
